Do you or any members of your CU have any hospitalization or health insurance plans or belong to a plan that pays all or part of your medical expenses? Include policies paid for someone outside your CU and please consider any special purpose plans you may have, such as those listed in the Information Booklet.

*Do not report Medicare Prescription Drug plans (Medicare Part D) here. Medicare Prescription Drug plans are collected in Section 14C in Interviews 2 through 5.
