Do not include fees for business purposes.

8 - OCCUPATIONAL EXPENSES, such as union dues or professional licenses.

HOME SERVICES PROVIDED BY NON-CU MEMBER

9 - GARDENING OR LAWN CARE SERVICES, including -

  fertilize lawn, etc.   planting   tree pruning
  hedge trimming   plowing   tree removal
  lawn cutting   tilling

Include any services provided under service contracts.

Do not include services which are covered by management or maintenance fees.

10 - HOUSEKEEPING SERVICES, including -

  carpet cleaning   cooking   window washing
  cleaning   laundering

11 - HOME SECURITY SYSTEM SERVICE FEES

12 - OTHER HOME SERVICES AND SMALL REPAIR JOBS AROUND THE HOUSE, NOT PREVIOUSLY REPORTED

Include diaper service.
